Required Forms
When the Required Forms are ready, you can download them from this page. When you see the forms below with ‘live links’, then they are ready for download. If the form says ‘PENDING’, then it is not available yet. You will be notified when it is available via email.
Once you complete your Required Forms, you can scan and upload them to our custom Participant Forms Management System (PForMS). There will be a button on this page for access to the PForMS system. Use the printable Checklist at the bottom of this page to keep track of your forms.
Note: We can ONLY accept scanned PDF forms that are legible and can be printed. No photos of forms or image files will be accepted. If you have questions, just ask us!

NOTE: All forms are for the Participant – either the Sea Scout or Adult attending the Cruise. If a youth (Sea Scout 14-17 years old), the parent/guardian must sign. If an adult (18 years old or above), the adult participant must sign. If you have questions, please ask before submitting the forms.
NOTE: You can upload forms in any order and upload them a few at a time. The PForMS system will save your uploaded forms as you go. You can return at any time to finish uploading additional forms. You will receive ‘status notifications’ via email when each form is under review (status = pending) and when it is approved (status = approved).
REQUIRED FORMS – for every participant, regardless of age.
1 AuxWaiver = USCG Auxiliary Photo Release – form 7020a here.
2 BoatingSafety = Boating Safety Course Completion Card (NASBLA). You can easily earn this card by attending classes:
USCG Auxiliary boating class information – in person class with demonstration of safety equipment.
BoatUS – the only free NASBLA class – online class information.
3 Consent = Activity Consent Form – PENDING
4 CruiseWaiver = Cruise Acknowledgement of Risk and Release – PENDING
5 DL = Driver’s License – of the person signing the CruiseWaiver form above.
6 HIC = Health Insurance Card that covers the participant. This is your medical insurance card.
7 PartAB = BSA Health Form, Parts A & B. Here is a link to Parts A, B & C. We use the Sea Base high adventure version.
8 PartC = BSA Health Form, Part C. Note: requires a physician signature – see link above for PartAB.
9 PubWaiver = Publicity Waiver and Release – PENDING
10 SwimTest = Swim Test (must show BSA “swimmer” classification). Swim Classification form.
REQUIRED FORMS – for any participant age 18 and above. For these items, go to My.Scouting.org and click on ‘My Training’.
11 HWeather = BSA Hazardous Weather.
12 SafeSwim = BSA Safe Swim Defense.
13 SAfloat = BSA Safety Afloat.
14 SYT = Safeguarding Youth Training Certificate.
OPTIONAL FORMS – these optional forms are not required to join ComCruise, but they are valuable.
15 ABC-USPS = America’s Boating Club (US Power Squadron) certificates
16 ARC_1stAid = American Red Cross First Aid
17 ARC_Lifeguard = American Red Cross Lifeguard
18 ASA = American Sailing Association certificates
19 BSA_Lifeguard = BSA Lifeguard Certification (now retired, but upload if you have it.)
20 Constellation = USS Constellation Overnight Form – Waiver Form here. Required only if you are doing this activity.
21 Fish = Fishing License (MD or VA) – only needed if you want to fish during ComCruise. A VA license can be registered in MD for no charge.
22 LNT = Leave No Trace Completion Certificate
23 Other = Any forms not listed above. Multiple “Other” form submissions are allowed.
24 US_Sailing = US Sailing Certifications
25 WFA = Wilderness First Aid certification – see our page on WFA for more details.
